Family Case Management is a person-centered program for people with developmental disabilities who live in Orange County. We are dedicated to helping you with the choices you have the right to make. We can help you find services that are available, and help you access these services.

Family Case Management will help you achieve valued outcomes because we promote:

Family Case Management is committed to the belief that ever individual should be included in the community to whatever extent they choose. Through creativity and dedication, we will help you explore possibilities and strategies that are beyond the standard procedures and services. And we look at consumer satisfaction as our number one priority.

Services Provided by Family Case Management:

• Program eligibility determination
• Referral and coordination of services
• Develop and implement an Individualized Service Plan
• Advocate for the consumer and encourage self-advocacy skills
• Maintain contact on a monthly basis with the consumer
• Provide assistance in dealing with various agencies: Social Security, OMRDD, VESID, Social Services, schools, Medicare, even private health insurance companies and hospitals.
• Encourage inclusion
• Help in emergencies and crisis
• Provide support

Family Case Management is an independent case management program. We take pride in our family-oriented focus because our organization was established and is overseen by families and professionals from Orange County. Since we are independent, we are not subject to the structures, restrictions and obligations of any agency. We realize that services are not always readily available, but we are committed to the goal that our case management program will provide consumer satisfaction.

We are aware of the advocacy work that is needed in the community in order to fulfill the specific plans and choices of the individual. We will explore possibilities and strategies, and reach for outcomes that are beyond the standard procedures and options of traditional agencies.

As a person-centered case management program, we are dedicated to knowing the personal preferences and desires of the individual. It is our intention to help them reflect on and improve their quality of life. We ensure that the person is satisfied with the supports and services that they receive. And, we will listen to the individual and help build a more positive future--one person at a time.
